Medina's chilling account began with her consumption of Xanax, a suboxone strip – a medication used in treating opioid addiction, and snorting three lines of cocaine on the evening before the incident. The following morning, Medina, her children, and her boyfriend began their day at approximately 6:30 a.m. As her boyfriend left for work, she remained alone with her two children, providing them with nourishment and engaging them in watching cartoons.

However, her day took a dark turn around 2:30 p.m., as she revealed that she heard voices in her head commanding her to "send her children to heaven today," or else they would face grave harm. Medina claimed that these voices threatened the torture of all three, herself included, if she failed to act.

She then brought her children into the bathroom, expressing her love for them with kisses before submerging them both under the water, gripping their necks until her young son ceased to have a pulse and both children stopped moving.
